Stem Cell Therapy: Which Claims Hold Up, and How Thailand Regulates It

A plain reading of stem cell therapy — what is approved, where the evidence for MSCs is strongest, and the Thai rules a compliant clinic has to follow.

Few areas of medicine carry a wider gap between what is proven and what is advertised. Stem cell therapy covers everything from a bone marrow transplant that has saved lives since the 1970s to an infusion sold for tiredness, and the same two words describe both. Sorting the categories is most of the work.

The approved list is short

Blood-forming stem cells — haematopoietic progenitor cells, from cord blood or bone marrow — are approved and have been routine for decades in disorders of the blood and immune system. That is genuine, established medicine.

In December 2024 the list grew by one: the FDA approved Ryoncil, the first mesenchymal stem cell therapy to reach approval, for steroid-refractory acute graft-versus-host disease in children from two months of age. It is a significant milestone and it is also narrow.

Outside those indications, mesenchymal stem cell treatment is investigational. That word does not mean useless, and it does not mean unethical. It means the studies that would establish it as standard care have not been completed. A clinic can offer an investigational treatment responsibly. What it cannot do is describe it as proven.

Where the evidence is genuinely encouraging

Knee osteoarthritis is where MSC research has gone furthest. Meta-analyses of randomised controlled trials report improvement in pain that is both statistically and clinically significant. Adipose-derived MSC therapy in particular has been described as safe, with effects that persist rather than fading within weeks, and researchers have raised the possibility of delaying joint replacement in some patients.

There is a caveat published in 2025 that deserves to be quoted rather than buried. A systematic review set out to quantify how much of the improvement after intra-articular MSC injection is attributable to contextual effects — the response people have to any injection, any attention, any expectation. A meaningful proportion was. That does not erase the treatment effect. It does mean a clinic showing you before-and-after pain scores without a control has not shown you what it thinks it has.

Preclinical work on MSC-derived exosomes for cartilage repair has been consistent across studies, which is a reasonable basis for continued research and not yet a basis for a clinical promise.

How Thailand regulates it

Thailand has a more developed framework here than many patients travelling for treatment realise, and knowing it turns “do I trust this clinic” into a set of checkable questions.

Cell therapies are treated as Advanced Therapy Medicinal Products under the Drug Act B.E. 2510, with ATMP guidelines issued in 2018 and updated in 2023. The Thai FDA oversees clinical research, manufacturing, GMP inspection and product authorisation. In 2025 the Department of Medical Sciences introduced national cell-bank standards covering donor eligibility, infectious disease screening and standardised processing.

Alongside the product rules sit the professional ones. The Medical Council of Thailand’s regulation on medical ethics for stem cell use requires that treatment be evidence-based or conducted within ethics-approved research, that physicians not present experimental therapies as proven, and that informed consent explicitly address investigational status, risks and alternatives.

Read that last requirement again, because it is the most useful sentence in this article. If a consent form does not say the treatment is investigational, either the treatment is one of the few that is approved, or the form is not doing what the regulation requires.

What to check before you commit

The honest summary

Mesenchymal stem cell therapy is a real field with real signal, most convincingly in joints, being offered widely for indications where the evidence is much thinner. Both halves of that sentence are true, and a clinic that only tells you the first half is selling rather than advising.

A clinic operating within the rules can answer all three without hesitation, because the rules already require it to have the answers.

Should I expect a cure?

No, and any clinic promising one is telling you something about itself. Realistic discussion is about symptoms and function — pain, mobility, what you can do again — over a defined follow-up period, with an agreed point at which you review whether it worked.
